Our Financial Policy
Office visits at Anew Health & Wellness are often covered by insurance. Each patient’s claims will be submitted to insurance but any remaining balance owed will be the responsibility of the patients. Payment is expected at the time of service and any remaining balances will be mailed out in monthly statements. Patients not submitting through insurance will receive a discount.
The fee schedule varies by appointment type and length. This information is available upon request.
Phone calls, answering messages through the secure Patient Portal, and requests for completion of medical forms are charged at the rate of $15 per 15 provider minutes spent, with a minimum of $15 per call/message/form.
Missed or Canceled Appointment
Appointments are a valuable resource. With the exception of sudden severe weather events that prevent safe travel to appointments, cancellations for new patient appointments must be communicated by phone to Anew Health & Wellness Reception (563-265-1341) at least 3 days in advance of the appointment time, and cancellations for follow up appointments must be communicated at least 24 hours in advance of the appointment time. Failure to cancel results in a $30 fee not reimbursable by insurance
